Packers Head Coaching Application (1999)

Guys, this came up in conversation with a fellow Packer buddy yesterday and thought I'd share. From late 1999 during/after the (only) season with Ray Rhodes as head coach. Don't remember where or how I got this, but I kept because I thought it was one of the funniest Packer-related things I've ever seen. For those that remember Ray Rhodes well, I think you'll enjoy.
